The funds are needed to perform a major surgery for an injury that happened at my former employer. Because I was instructed by my physician to have around the clock care for at least 7months post surgery. Due to being terminated from job due to work related injury and has last received work Comp payments 2nd week of Oct 2025, I now no longer can afford housing while still in need of major surgery..

This would mean a great deal for me and my family because I depend on them a lot because I can't do normal things anymore as I have been diagnosed with a tear in my rotator cuff and been recommended Arthroscopy surgery back in Mar 2026. My adjusters has not return calls/emails about the surgery I tried to notify them about. Instead I would get switched to a new adjuster and still no response.

I have a breakdown in Dr/patient relationship. I no longer trust my physician nor am I comfortable with him. He didn't tell me off the surgery in a way where I could ask questions of concern. He just said he recommended the type of surgery and handed me a package detailing the type of surgery. Upon next appointment (3wks later) Because I didn't know how to approach my questions/concerns I told him that Google said I.dont necessarily need surgery to recover.(Remnder : I have a ton of quetions but couldn't ask him til this apointment ) Thinking he was going to be more detailed about the surgery and correcting me that Google isn't accurate or something of that nature. He instead threatened me to either have the surgery or go back to work full duty. (Reminder: I wasn't able to work full duty with the lesser previous diagnosis) No other options given. Then proceeded to say that he is a private Dr and he shouldn't have to basically be bothered by seeing me. On top of that he has changed my meds and as I was reading the new medicine it stated not to take it if A-i take Naproxen (and have for a year) or B- if im a smoker ( and was but not as much due to trying to quit). He has constantly offered a kind of meds that I previously (through a previous Dr) have taken and had an adverse affect (causing bleeding) yet every month I go to see him he still offers me those same meds that I had experience a bad side affect even after repeatedly telling him each visit I cannot take those. He never seems to read my chart first before offering those meds. For me that was an unsafe practice. So this is where I stand with the work Comp Dr. Finally got an adjuster to return my call/email but only after I stated I would like to close my medical (because they still haven't responded for my help in nearly 5mths for the surgery)and was offered $5000 when they did finally responded. This would only cover a portion of my physical therapy after surgery. It doesn't even gets me on the operating table. Then the adjuster stated the Dr said I don't need surgery(late July 2026- after my constant cries for help with advice for the surgery  nearly 5 months earlier )again feeling more unsafe because he documented that I need surgery and recommended the type of surgery earlier So now I don't trust, nor comfortable, nor safe with the adjuster as well now  Even request for a new panel of Physician and was given a HARD no. Its even documented with work Comp Bureau and original adjuster of the breakdown with the Dr and myself. Even told them I would go through with the surgery if appointed a different Dr. Still a HARD no. Im scared! So I'm reaching out for help to get this surgery through a Dr of my choice that isn't under the work Comp umbrella. And by the way they are not trying to negotiate the $5000 offer which will not get me the surgery and after care I would need pertaining to my work related injury.
